Overview:

Sentara Home Care is seeking to hire a qualified individual to join our team as a Care Transition Coordinator.

Position Status: Full Time - Day Shift

Position Location: Chesapeake, VA

Hourly Range: $28.16/hr - $46/hr

Overview 

Coordinates and directs the referral activities from health care facilities to home care by identifying/determining appropriateness of care, gathering medical and financial data, meeting with patients and families, and working with care management, physicians and referral sources to ensure a smooth transition. Responsible for safe and effective transfer from facility to home, and securing patient referrals by maintaining customer relationships.

  • Collaboration with internal teams 

  • Patient review of appropriateness for services 

  • Need identification and coordination of services to meet patient need 

  • Physician order obtainment 

  • Patient and family management 

Education 

  • High School Diploma (Required) 

 

Certification/Licensure 

 

  • Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI 

  • Valid license in state of practice required 

  • Licensed Practical Nurse (LPN) Multi State - Nursing License - Virginia Department of Health Professionals (VADHP) or Licensed Practical Nurse (LPN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP) or Licensed Practical Nurse (LPN) Multi State - Nursing License - North Carolina Board of Nursing (NCBON) or Licensed Practical Nurse (LPN) Single State - Nursing License - North Carolina Board of Nursing (NCBON) or 

  • Registered Nurse (RN) Multi State - Nursing License - Virginia Department of Health Professionals (VADHP) or Registered Nurse (RN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP) or Registered Nurse (RN) Multi State - Nursing License - North Carolina Board of Nursing (NCBON) or Registered Nurse (RN) Single State - Nursing License - North Carolina Board of Nursing (NCBON) or 

  • Physical Therapist Assistant License (PTA) - State license - North Carolina Board of Physical Therapy Examiners (NCPT) or Physical Therapist Assistant License (PTA) - State license - Physical Therapy Compact Commission (PTCOMPACT) or Physical Therapist Assistant License (PTA) - State license - Physical Therapy Compact Commission (PTCOMPACT) or 

  • Respiratory Therapist License - State license - Virginia Department of Health Professionals (VADHP) or 
    Respiratory Therapist License - State license - North Carolina Respiratory Care Board (NCRCB) or 

  • Licensed Master Social Worker (LMSW) - State license - Virginia Department of Health Professionals (VADHP) or Certified Master Social Worker (CMSW) - State license - North Carolina Social Work Certification and Licensure Board (NCSWCLB) or Social Worker (CSW) - State License - North Carolina Social Work Certification and Licensure Board (NCSWCLB) 

 

Experience 

  • 1 year of home care, case management or discharge planning experience (Required).
