Care Specialist

Essential Duties and Responsibilities:

Client Start 

● First caregiver with client new starts 

  • Introduction of the client and caregiver, explanation of the Care Specialist role, and the value added to client care

  • Setting TheKey caregiver high standard of care and expectations 

  • Meet with Care Manager after the first shift to give feedback and recommendations for care plan updates

● Provide insight on caregiver matching - traits and client needs shared with CM 

● Care Management binder maintenance - ensure medication sheets and blue care notes are in place, PPE supplies are available, and binder is in an easy accessible location for caregiver and client to use 

Caregiver Start/Training 

● Client Care Plan Expert

● Coordinate caregiver introductions, training at client residence, and ongoing training as needed with Caregiver Leads 

● New client - CM owns the original training and will have CS attend to set the expectations 

● Stabilized client (12 weeks or more) - CS will own ongoing hands on training and introductions

  • Review and Check understanding of knowledge of Care Plan in detail with caregiver and train on specific care needs (text only as a follow up) 

  • Reinforcing CM is the main point of contact for all Caregivers and Clients 

● Lead Caregiver program - identify, recruit, and train the Lead Caregiver program with every client 

  • Utilize Lead Caregiver checklist 

Retention 

● Deliver supplies/update binders as directed by CM 

● Prioritize with CM on a daily basis at risk clients, caregiver additional training, and other related tasks that CM's may feel would increase quality of care and communication with caregivers 

● CM communicates with CS all Medallia client surveys and works together to improve outcomes

Scheduling 

● Set up check in for brand new caregivers to TheKey after their first shifts.

● Filling in as a Caregiver when last minute call offs or NCNS occur - covering only until a replacement arrives

● May be required to work a start of care shift as necessary to assure client care is off to a good start

● Assisting Care Management or Care Specialist in Lincoln on a temporary basis, as needed

● Weekly give overview to Director of Client Care - measures time & effort 

Attire 

● Scrubs will be provided (3 shirts and 2 pants) or dark jeans (with no holes or tears) and TheKey t-shirt 

● Close toed shoes, sturdy, slip resistant, and appropriate for client care 

● Name Badge provided and worn for duration of the shift

Work Schedule & Expectations 

● Monday thru Friday 9a to 5p is the regular schedule 

  • At times a Care Specialist will be asked to work outside of the regular schedule for the above tasks. If this occurs, the schedule will be adjusted to stay under 40 hours for each pay period (Monday-Sunday)

  • A lunch break of 30 minutes will be given daily, unless working with a client

● Care Specialists are expected to work any Holidays that fall during the week - Holidays that are worked will be paid at time and a half

● Care Specialist will log hours by clocking in and clocking out via Workday - any incorrect clock ins/outs need to be reported to the manager within 24 hours of error

● Time off - any time off needs to be requested off via Workday and approved by the Director of Client Care

● Clocking in and out of all scheduled client shifts using either the WellSky Personal Care app or Telephony is required - Any incorrect clock ins/outs need to be reported to the manager within 24 hours of error

● All mileage will be recorded using Coupa.

Required Skills, Education and Certifications:

  • 1-3 plus years of experience in health care, elder care, social work or related industry 

  • Excellent customer service and communication skills 

  • Computer proficiency and ability to document timely and accurately in systems

  • Current driver’s license and proof of insurance 

Preferred Skills, Education and Certifications:

  • Extensive knowledge of industry regulations and governing laws

Physical Requirements:

  • Ability to travel approximately up to60% of the time

  • Ability to lift and carry up to 75 pounds

  • Ability to sit, stand and walk for prolonged period of time throughout the work day

  • Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
