Works as a member of the multidisciplinary team to provide support for the discharge process and complex patient needs. Performs follow up tasks identified by the Care Managers for management of patients across the continuum.
Education: High school diploma with three years of related experience and must obtain a Bachelor’s degree within two years or Bachelor's degree with one year of related experience or Bachelor's Social Work (BSW) degree.
Experience: Three years of related experience in an acute care setting with knowledge of discharge process and post-acute care with high school diploma or one year related experience in an acute care setting with knowledge of discharge process and post-acute care with Bachelor’s degree. No experience required with BSW.

Knowledge and Skills: Exceptional Customer Service skills. Sets priorities and gets desired results. Possesses strong verbal and written communication skills. Self-directed and able to organize and manage multiple tasks/projects simutaneously in a fast-past, deadline driven environment. Ability to promote and build a multi-disciplinary care concept. Understands HIPAA and importance of privacy of patient data.
Special Training: Proficient in Microsoft Office Suite, specifically Word, Excel, Outlook, and general working knowledge of Internet for business use. Spanish as a second language is a plus.

Usual Work Day:8 Hours
Gathers information regarding patient discharge status and updates team members.
Assists in facilitating discharge of patients from the hospital to home or extended care facilities by coordinating various processes and tasks related to discharge planning and resource utilization.
Provides and educates families with “Medicare 2nd Notice” letters as directed by Care Managers.
Follows complex discharge patients in post acute setting to ensure contract terms are being met.
Collaborates with Care Managers and Social Workers by preparing discharge packets for patient transfer and assures the completeness of materials.
Attends interdisciplinary rounds, for the purposes of discharge planning, data collection and reporting as needed.
Assists with special programs including data collection for process improvement projects.
Completes a variety of department specific tasks to assist with optimal department safety and functioning.
Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
Performs other duties as assigned.
