Territory Assignment:

This is a field-based role supporting members across Southern Cook County, Illinois. Candidates should reside centrally within Southern Cook County to ensure timely access to members for in-person case management activities. Travel throughout the assigned territory is required to conduct home visits, attend interdisciplinary care team meetings, collaborate with providers, and engage with community resources.

Position Summary:

The Care Manager RN applies a collaborative process of assessment, planning, facilitation, and advocacy to meet members’ health needs. Responsibilities include coordinating services, evaluating outcomes, and promoting wellness and cost-effective care. The role involves both telephonic and in-person engagement, with a strong emphasis on field-based case management.

Key Responsibilities:

  • Conduct in-home assessments and care coordination visits within Southern Cook County and surrounding areas.
  • Develop individualized care plans addressing medical, behavioral, and social needs.
  • Collaborate with providers, caregivers, and community organizations.
  • Monitor member progress and adjust care plans as needed.
  • Document case management activities in compliance with regulatory and company standards.
  • Participate in interdisciplinary team meetings and contribute to integrated care strategies.

Travel Expectations:

  • Regular travel throughout Southern Cook County is required.
  • Reliable transportation and a valid driver’s license are necessary.
  • Mileage reimbursement provided per company policy.

Required Qualifications:

  • Active, unrestricted RN license in the state of Illinois.
  • Minimum 3–5 years of clinical experience.
  • At least 2 years of experience in case management, discharge planning, or home health coordination.
  • Comfortable working independently and virtually.
  • Proficient in Microsoft Office Suite and virtual collaboration tools (Teams, Outlook, etc.).
  • Strong communication, organizational, and problem-solving skills.

Preferred Qualifications:

  • BSN degree.
  • Certified Case Manager (CCM) credential.
  • Experience working with dual-eligible populations or in managed care settings.

Pay Range

The typical pay range for this role is:

$66,575.00 - $142,576.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
