Elder Life Management, now part of the BAYADA family, is seeking a compassionate Care Manager to support our elder clients in Tinton Falls, NJ, and surrounding areas. 

This role requires the ability to assess, plan, and coordinate care for older adults both at home and in facility settings, while advocating for their independence and well-being.

As a Care Manager, you will: 

  • Conduct comprehensive assessments and develop individualized care plans
  • Coordinate home care services and other supports
  • Attend medical appointments as a client advocate and provide ongoing support
  • Act as a liaison for families at a distance
  • Arrange, monitor, and adjust in-home services as needed
  • Provide crisis intervention and problem-solving support
  • Engage and coordinate community resources to enhance clients’ quality of life
  • Promote meaningful social, spiritual, and cultural activities
  • Maintain accurate records and documentation of client care

Qualifications:

  • Degree in Nursing or Social Work required; equivalent experience considered
  • Experience in geriatrics or elder care preferred
  • Strong interpersonal and communication skills
  • Ability to work independently while collaborating with families and service providers
  • Valid driver’s license; company car provided for work-related travel
