Environmental Modifications Care Management Associate (EM CMA)

The Environmental Modifications Care Management Associate (EM CMA) plays a key role in supporting Long‑Term Services and Supports (LTSS) members by coordinating home modification projects that improve safety, accessibility, and quality of life. In this role, you’ll serve as the primary point of coordination for Environmental Modification (EM) requests—from initial referral through project completion.

You’ll partner with Care Managers, contractors, Medical Directors, and members to ensure projects are delivered efficiently, compliantly, and with a strong focus on member satisfaction. This position is ideal for someone who enjoys project coordination, vendor collaboration, and mission‑driven work.

What You’ll Do

  • Manage EM requests from referral through completion

  • Coordinate with vendors and contractors, including review of bids and scopes of work

  • Support authorization and utilization workflows

  • Ensure compliance with Medicaid, LTSS, and waiver requirements

  • Communicate clearly with internal teams and members to keep projects on track

Required Qualifications

  • Experience in Medicaid, LTSS, waiver programs, or healthcare administrative support

  • Experience with authorizations, utilization management, or vendor coordination

  • Ability to review contractor bids, pricing, and scopes of work

  • Strong organizational, time‑management, and communication skills

  • High School Diploma or equivalent

Preferred Qualifications

  • Experience with Environmental Modifications or HCBS programs

  • Knowledge of state Medicaid policies and medical necessity criteria

  • Experience working with contractors or vendors

  • Proficiency with utilization management systems and Microsoft Office

  • Associate degree

Pay Range

The typical pay range for this role is:

$18.50 - $35.29

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
