Key Position Details:

Job Description:
Provides clerical and support functions to the social work and case management staff members in the areas of clinical information, transportation, durable medical equipment (MDE)/oxygen needs, etc. with the goal of promoting expeditious patient transfers and/or discharges. This is accomplished through the major job components of coordination/communication, delegated responsibilities, role modeling, quality/resource stewardship and patient advocacy/customer service.
Principle Responsibilities
- Clerical Service Coordination
- Patient transfer and discharge is supported efficiently and effectively, prioritizing post acute discharges including, but not limited to, skilled nursing homes and home health care.
- Assists with preparing the required patient medical record information for discharge to nursing homes, home care hospice or other medical facility.
- Exchange of protected health information is sent accurately via fax, telephone, hard copy, etc.
- Contacts nursing homes to determine bed availability and communicates information to department staff.
- Assists with arranging transportation as requested.
- Assists with medication authorization, taxi vouchers etc. as requested by department staff.
- Effectively communicates and exchanges information to appropriate personnel, department or outside agency to promote continued progression of plans of care and discharge.
- Department and patient needs are prioritized and implemented according to unit/hospital policies, procedures and standards.
- Interpersonal communication with both internal and external customers is courteous, responsive and timely.
- Assists with data collection and reporting projects as assigned.
- Patient Advocacy/Customer Service
- Provides assistance to the department when Admin Assistant is absent i.e. phone and reception duties, sorting of mail etc.
- Partners with other Care Management Department staff members to promote timely discharges.
- Verbal, electronic and telephonic communication are consistently demonstrated at an excellent service level.
- Requests for assistance are responded to in a timely, efficient and positive way.
- Confidentiality of patients, families, and other staff is maintained at all times.
- Communication with staff and other key stakeholders are timely, understandable and responsive to needs and questions.
- Establishes positive working relationships with community resources.
- Quality Resource/Stewardship
- Utilizes resources effectively and efficiently.
- Participates in quality improvement activities to evaluate and improve systems and processes that support patient care.
- Effective time utilization and organizational skills are applied to achieve productivity and promote efficient utilization of resources.
- Cost effective use of supplies and equipment in providing quality care is demonstrated.
- Flexibility and creativity is demonstrated in response to meeting the changing patient, Department and staff needs.
- Quality concerns/suggestions for positive change are identified and communicated.
- Changes based on quality improvement results are incorporated into work.
- Safe work practices are encouraged and demonstrated to prevent injury to self, peers and/or patients.
- Other duties as assigned.
Required Qualifications
- Must be 18 years of age with education and/or experience needed to meet required principle competencies as listed on the job description
- Ability to Prioritize and organize tasks with attention to detail in a fast paced environment
- Knowledge of medical terminology
- Ability to establish effective relationships with health care team members
- Excellent verbal communication and telephone skills
- Proficient computer and clerical skills and familiarity with/ability to navigate in clinical, business information systems, and data bases
- Excellent customer service skills with a people oriented philosophy of service
Preferred Qualifications
- High School or GED
- Associate's or Vocational degree
- 1 year of experience in an acute care hospital setting

- Lifting weight up to 10 lbs. occasionally, negligible weight frequently
Pay Range
Pay Range: $20.32 to $27.68 per hour
The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
