  • RN Required (MO and KS)

  • Position onsite at Wright Memorial, but may also support Hedrick Medical Center and KS locations remotely

  • Proficient computer skills and quick learner highly preferred

The Opportunity:

Responsible for the coordination of services through the continuum of care with the cost effective utilization of resources. Performs clinical review to determine appropriateness of admission appropriate level of care and use of clinical resources in the acute hospital setting. Supports collaboration and communication with patient family physician and healthcare team regarding the plan of care and post discharge needs with facilitation of services required. For Home Care and Hospice, serves in a consultant role, acting as an advocate for the Saint Luke's Home Care and Hospice programs, providing education for healthcare professionals and families and facilitating continuance of medical care from inpatient care to home care for post acutely and chronically ill patients and their families.
