Are you highly organized, people‑focused, and passionate about supporting safe and timely patient discharges? Join our Care Coordination team as a Care Coordination Assistant, where you’ll play a vital behind‑the‑scenes role in helping patients transition smoothly from the medical center to the next level of care. This role is ideal for someone who thrives in a fast‑paced healthcare environment, enjoys collaboration, and takes pride in attention to detail.

Essential Functions

  • Collaborate with Care Coordination staff, medical center teams, and outside agencies to ensure timely, safe, and appropriate patient discharges
  • Assist with the implementation of discharge plans by:
    • Preparing discharge packets
    • Providing current community resource lists
    • Initiating FL2s and PASRRs as needed
    • Researching resources to meet identified discharge needs
  • Arrange concrete discharge services at the direction of the Case Manager and/or Social Worker, including:
    • Durable Medical Equipment (DME)
    • Home health services
    • Transportation (ambulances, taxis, etc.)
    • Assisted Living Facility (ALF) and Skilled Nursing Facility (SNF) placements
  • Support efficient workflow by assisting with:
    • Clothing closet maintenance
    • Guardianship application processes
    • Emergency Department staff support
    • Basic computer hardware and software needs
  • Ensure delivery of Medicare Important Message (IM) letters to all Medicare and managed Medicare patients within required timeframes
  • Facilitate smooth exchange of information with internal departments and external agencies
  • Complete ad hoc reports, filing, data entry, and general clerical duties while prioritizing tasks to meet leadership deadlines
  • Assist with audits of case management services and compile findings that support quality and effectiveness metrics
  • Perform reception duties, including answering phones, responding to questions, and assisting patients and visitors in accordance with departmental standards

Education, Experience, Licensure & Certifications

  • High school diploma or GED required
  • Associate’s degree in a health‑related field preferred
  • Two (2) years of office or administrative experience preferred
  • Licensure, certification, or registration: Not required

Skills & Qualifications

  • Excellent oral, written, and interpersonal communication skills
  • Strong customer service skills with the ability to interact courteously and compassionately with the public
  • High attention to detail and accuracy
  • Proficiency in data entry and typing
  • Strong organizational and time‑management skills
  • Ability to manage high call volumes, frequent interruptions, and customer traffic
  • Ability to work effectively as part of a multidisciplinary healthcare team
  • Experience using standard office equipment
  • Working knowledge of Windows, Excel, and Outlook preferred
