Overview
The Cardiovascular Community Health Worker bridges healthcare and the community by coordinating care, educating patients, and connecting them with essential resources. They act as a bridge between providers and the community, helping reduce socioeconomic barriers and advocating for patients. Their efforts focus on improving cardiovascular health outcomes through access, education, and relationship building.

What you will do

• Facilitate patient access to various community resources while collaborating with social services agencies and medical providers.
• Educate patients on health management, the appropriate use of healthcare services (e.g., emergency room alternatives), and encourage active participation in their health and well-being.
• Work closely with an interdisciplinary team, including Physicians, Nurses, Care Managers, Social Workers, and Health Educators and others, to improve patient health outcomes and address care gaps.
• Conduct telephonic outreach to engage members in care, address service coordination needs, close care gaps, and connect members to educational and community support programs.
• Maintain accurate and comprehensive records of client interactions, service plans, and outcomes in electronic health records, submitting monthly reports as required.
• Build positive relationships with clients, providers, and community organizations, working to reduce cultural and socio-economic barriers that impact access to care.
• Participate in community events and activities to foster relationships, understand community health issues, and promote awareness of available resources.

Qualifications

Education

• High School Diploma or GED, (Required)
• Associates Degree, (Preferred)

Experience

• 1 year Healthcare related experience (Preferred)

Licenses and Certifications

• Certified Nurse Aid (CNA) - Virginia Department of Health Professions (VDHP) (Preferred)
• Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) (Preferred)
• Valid Drivers License Required
