The Birth Certificate Clerk will be responsible for obtaining correct information for birth certificate reporting and preparing the certificate in accordance with State of Utah Vital Statistics Rules and Regulations.  Coordinate and organize birth certificate information to ensure smooth workflow and provide quality assurance within the Birth Certificate process.  Gather material and ensure timely transport to Vital Statistics.  Responsible for gathering statistical information for Health Information Management.

Here’s what you will experience working as a Birth Certificate Clerk

  • Interviews obstetrical patients in order to complete the worksheets for the necessary information required on the birth certificate.

  • Reviews the medical records of mother and baby to accurately complete the health section of the certificate via paper and electronic record, "Epic."

  • Tactfully informs the mother and/or father of any necessary reporting and charge requirements in the completion and/or correction of a birth certificate.

  • Types birth certificate accurately for every newborn in accordance with State of Utah Vital Statistics rules and regulations.

  • Forwards accurate and complete birth certificates to the Salt Lake County Health Department within eight-day time requirement.

  • Flags all adoption recordings according to the policy.

  • Provides patient with verification of birth as requested for financial assistance or other legitimate purposes.

  •  Gather statistics and report to the Management Team.

  • Performs other duties as assigned by the supervisor.
     

Required Skills:

  • High School Diploma

  • Prior experience in HIM Department preferred. Computer system experience desirable.

  • Knowledgeable of Health Information Management organization and functions and computerized medical records applications desirable.

  • Knowledgeable of State Rules and Regulations as they apply to birth certificate process desirable.

For this US-based position, the base pay range is $19.26 - $32.11 per hour . Individual pay is determined by role, level, location, job-related skills, experience, and relevant education or training.
