Summary
The Hospitalist Nurse Practitioner provides advanced nursing care to hospitalized patients, working collaboratively with physicians, nurses, and other healthcare professionals to manage acute and chronic conditions. The NP performs comprehensive assessments, develops and implements treatment plans, orders and interprets diagnostic tests, and coordinates patient care to ensure optimal outcomes during hospital stays.Job Description
Perform comprehensive patient assessments, including history, physical exams, and review of diagnostic results. Diagnose and manage acute and chronic medical conditions in hospitalized patients. Develop, implement, and modify individualized treatment plans in collaboration with the healthcare team. Order, interpret, and follow up on laboratory tests, imaging studies, and other diagnostic procedures. Prescribe medications and treatments according to hospital protocols and state regulations. Monitor patient progress and adjust care plans as necessary. Facilitate patient and family education regarding diagnosis, treatment, and discharge plans. Coordinate patient care with consulting specialists, nursing staff, social workers, and other healthcare providers. Document all patient encounters and care plans accurately and timely in the electronic medical record (EMR). Participate in hospital rounds, quality improvement initiatives, and staff education. Adhere to hospital policies, procedures, and regulatory requirements. Respond to emergent patient needs and assist in critical care situations as needed. This position also provides on-call coverage outside of regularly scheduled hours.Qualifications
