APP MEDICAL ADMITTING CLINICIAN (MAC) 

 

Join a dynamic team of Nurses and APPs driving patient movement across multiple campuses.  The key areas of focus are the Emergency Department, PACU, ICUs, Direct Admissions, and Transfer Center patients.  

Responsible for coordinating hospital patient flow including patient transfers to NYP Hospital both domestic and international.  Works collaboratively with all members of the team of the assigned center and units to ensure high level patient care, and patient satisfaction.   

Essential Job Duties  

All transfers to NYP Hospital will follow the Inter-hospital Transfer Policy. Obtain clinical information for all transfer requests from requestor. Triages case to the appropriate service line attending. Collaborates with admitting teams and departmental leadership to facilitate patient admission.  This includes identifying the appropriate level of care for admission, the patients' appropriate needs, and identifying and facilitation of admission to the appropriate part of the medical service in consonance with this. Coordination of communication amongst the patient care team. 

A Medical Admitting Clinician plays a crucial role in managing patient flow by overseeing the admission process. They assess patients upon arrival to determine the appropriate level of care and ensure that they are directed to the correct department. This involves evaluating medical histories, current health status, and any urgent needs. They work closely with other healthcare professionals to streamline admissions, reduce waiting times, and ensure that patients receive timely and appropriate care. 

Responsibilities include but not limited to:

  • Documents patient level of care requirements and attributes for proper patient placement in bed assignment software, updates accordingly.  Prioritizes cases accordingly. 

  • Collaborates with admitting teams and Chief Resident to ensure patient/physician assignment. 

  • Maintains communication with the referring facility regarding patient transfer status and condition. Provides updates to the NYP team regarding changes in patient condition. Maintains communication with all service providers. Coordinates caregivers sign out prior to transfer. 

  • Evaluates each patient transfer to monitor, assess and improve the efficiency and overall quality of the transfer process. Identifies barriers to patient transfer. 

  • Participates in data review and QI projects. Analyzes and provides reports to key management staff on a regular basis.  

  • Assures accurate and complete documentation in the Transfer Center data base for all transfers. Documents appropriately reasons for transfer delays, denials, cancelations and lost cases. 

  • Oversees departmental functions in the absence of the management team, coordinates communication with administration.  Maintains communication with administration with regular updates as needed. 

  • Maintains working knowledge of departmental guidelines and procedures. 

  • Ensure appropriate bed request in EPIC and honoring request.

  • Maintains working knowledge of departmental guidelines and procedures. 

  • Performs other job-related duties as assigned. 

Shift: (3) 12.5-hour rotating shifts per week. Nights, weekend and holiday rotation included.

Required Criteria:

  • Degree or major in Certified Physician Assistant or Master's in Nursing APRN (NURSE PRACTITIONER)

  • Licensure as a Physician's Assistant by the State of New York

  • Certification from the NCCPA.

  • Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS)

OR

  • Licensure as a Nurse Practitioner

  • Current NYS RN Licensure for Advance Practice Register Nurse

  • Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS)

Preferred Criteria:

  • Minimum of five years of clinical experience as a Physician Assistant/Nurse Practitioner working in Emergency Department, ICU, or Hospital Medicine

  • Knowledge of hospital operations

  • Experience with EPIC EHR

  • Strong Inter-Personal Skills
