Position Purpose
Ambulatory Care Pharmacists provide comprehensive medication management for a wide variety of patients with chronic health conditions. This position supports our Infectious Disease and GI Clinics by providing chronic medication management under a collaborative practice agreement. Areas of focus include but are not limited to HIV and Hepatitis C therapy.
What you will do
Precept multiple learners including IPPE, APPE, PGY1 and PGY2 Pharmacy Residents
Provide medication information to medical residents, fellows and attending physicians as related to the area of practice
Apply critical thinking to develop safe and effective therapy plans in real-time to implement during patient appointments
Order prescriptions and labs as outlined in the Collaborative Drug Therapy Management (CDTM) Agreement
Identify opportunities to expand and improve clinical pharmacy ambulatory services throughout the organization
Minimum Qualifications
B.S. degree in Pharmacy or Doctor of Pharmacy Degree
PGY2 Ambulatory Care Residency training or equivalent experience preferred
Current Connecticut Pharmacist License
Board Certification preferred, would be required to obtain BCACP credentialing within one year of eligibility
