Position Purpose

Ambulatory Care Pharmacists provide comprehensive medication management for a wide variety of patients with chronic health conditions. This position supports our Infectious Disease and GI Clinics by providing chronic medication management under a collaborative practice agreement. Areas of focus include but are not limited to HIV and Hepatitis C therapy.

What you will do

  • Precept multiple learners including IPPE, APPE, PGY1 and PGY2 Pharmacy Residents

  • Provide medication information to medical residents, fellows and attending physicians as related to the area of practice

  • Apply critical thinking to develop safe and effective therapy plans in real-time to implement during patient appointments

  • Order prescriptions and labs as outlined in the Collaborative Drug Therapy Management (CDTM) Agreement

  • Identify opportunities to expand and improve clinical pharmacy ambulatory services throughout the organization

Minimum Qualifications

  • B.S. degree in Pharmacy or Doctor of Pharmacy Degree

  • PGY2 Ambulatory Care Residency training or equivalent experience preferred

  • Current Connecticut Pharmacist License

  • Board Certification preferred, would be required to obtain BCACP credentialing within one year of eligibility
