This position will be employed by the Greater Rochester Independent Practice Association, INC. (GRIPA). GRIPA was founded in 1996 as a unique collaboration between hospitals and physicians, for the purpose of simultaneously improving the quality and efficiency of health care. This position will not be employed by Rochester Regional Health (RRH).
SUMMARY
An Ambulatory Care Clinical Pharmacist is responsible for promoting rational, cost-effective prescribing of drug therapy by working with patients and physicians (both GRIPA and non-GRIPA providers). The Pharmacist will also enhance and support patient education activities.
RESPONSIBILITIES:
Evaluate appropriateness and cost-effectiveness of drug therapy and identify and resolve medication related issues.
Design and individualize drug and non-drug therapies in the management of ambulatory disease states.
Provide drug information and pharmacy resources to physicians and their staff, and other healthcare professionals as deemed necessary and appropriate, or as requested.
Coordination of patient care: Facilitates a team approach and fosters a positive working relationship with members of the health care team.
Works in collaboration with patient/caregiver, primary care physician, and all pertinent members of the healthcare team to assess, facilitate, plan, and advocate for quality, cost-effective, and appropriate services to meet the patient’s health needs on an individual basis.
Participate in face-to-face encounters with patients in their home, physician office and/or other setting, as needed.
Facilitate quality, cost-effective medical and pharmacy management.
Monitor their effectiveness in decreasing preventative hospitalizations and ER visits and other unnecessary use of health care resources.
Report on successes as well as areas where there are opportunities for improvement.
Maintain current knowledge base relative to clinical practice, disease management, and literature for the development of pathways and assessment tools to identify high risk care needs related to pharmacy.
REQUIRED QUALIFICATIONS:
Doctorate Degree in Pharmacy preferred or an equivalent combination of education and clinical pharmacy experience.
Pharmacy license in New York State.
Five (5) years Ambulatory Care Clinical Pharmacist experience, Clinical Long-Term Care and/or Acute Hospital Clinical Pharmacist experience and/or Pharmacy Residency in Ambulatory Care.
Strong computer skills.

Independent means of transportation, and a valid driver’s license.
PREFERRED QUALIFICATIONS:
Board certification in Pharmacotherapy and/or Ambulatory Care.
Board certified Geriatric Pharmacist.
Bilingual skills are preferred.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
