Summary:

Bachelor's degree (or work experience equivalent) in related field, such as but not limited to, Education, Social Work, Public Health or Healthcare Administration. 

The AHVN Home Visitor is responsible for providing home visiting services in a home-based model, which includes a family educational program designed to assist parents in the areas of parent-child interaction, home safety, child health, and school readiness.

Description

1. Identifies, recruits, and enrolls families in home visiting services; ensuring family eligibility requirements are met according to individual grant and model fidelity requirements.

2. Administers assessments required by program and assists in the screening and preparation of children and parents to enable families to fully participate in home visiting services and activities and meet their family goals.

3. Drives to family homes delivering home-based activities and curriculum that enhance social/emotional, physical, and intellectual development while also delivering parent education.

4. Assists with planning and preparation of and attends planned meetings, monthly group meetings, and recruitment events, and participates in all regular sponsored activities.

5. Makes referrals to other community resources as appropriate.

6. Maintains accurate written and electronic records/files including: screening instruments, assessments, and all other documentation required by funding source in ETO and/or other required databases.

7. Performs other duties as assigned.
