Palliative CareOverview:
Position Highlights:
Monday–Friday; 8:00 a.m. – 4:30 p.m.
Performing outpatient palliative care consultations and follow-up visits for patients with complex symptom management and advanced care needs
Facilitating family meetings focused on medical decision-making, advance care planning, and goals of care discussions
Collaborating closely with referring oncology providers, social workers, and other interdisciplinary team members to ensure coordinated, holistic, and patient-centered care
Supporting seamless transitions of care, including coordination with home-based palliative care or hospice programs as appropriate
Participating in team meetings to promote best practices in symptom management, emotional support, and quality of life improvement for patients and families
