Advanced Practice Provider General Practice Mission Farms

Job Description​

We are hiring an experienced APP for our Mission Farms Primary Care Clinic in Overland Park, KS. This is a full time role, Monday-Friday, 4x10 shift. No weekend rotation. Some holidays and call rotations.

• This position is accountable for advanced nursing practice in the care of assigned patient populations
• Responsible for collecting and analyzing client health data
• Identifies appropriate expected outcomes
• Develops and implements patient plan of care
• Provides comprehensive clinical coordination of care and case management
• Provides consultation to influence plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulations
• Evaluates clients progress in attaining expected outcomes
• Conducts nursing research and provides education to patient family and nursing staff
