Virtua Health is seeking a Heart Failure Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our collaborative and highly specialized Heart Failure team. About the Role
As a Heart Failure APP, you will partner closely with cardiologists and the heart failure care team to manage complex patients across the outpatient continuum. You’ll play a critical role in optimizing therapy, educating patients, and improving outcomes for individuals living with heart failure.
This position is ideal for an APP who enjoys clinical autonomy, teamwork, and long-term patient relationships.

What You’ll Do
•    Manage adult patients with heart failure in the outpatient setting
•    Perform comprehensive assessments, follow-ups, and ongoing disease management
•    Initiate and titrate guideline-directed medical therapy
•    Educate patients and families on heart failure self-management and lifestyle modification
•    Collaborate with cardiologists, nurses, pharmacists, and care coordinators
•    Support quality initiatives focused on reducing readmissions and improving outcomes
•    Document care efficiently and accurately in the electronic medical record

What We’re Looking For
•    Nurse Practitioner or Physician Assistant with cardiology experience specifically in heart failure is strongly preferred
•    NP or PA with passion for chronic disease management and patient education
•    Strong clinical judgment, communication skills, and team-based mindset

Compensation:
•    The starting salary for this position is: $125,000.00 (annually).
The actual compensation package could vary based on factors such as, but not limited to, the applicant’s experience, internal equity, and alignment with market data.
