The Advanced Practice Clinician provides care via telehealth five days per week and serves on an as-needed basis as support to the centralized follow-up team. PPGP requires all staff members, including those who do not serve in patient-facing roles at our health centers, to work onsite at one of our facilities once per week to facilitate meetings and promote teamwork. As a result, the telehealth APC work schedule allows for work to be performed four days per week at a remote work site and one day per week onsite at the health center closest to their residence.
The APC will function as a provider of medical services for PPGP patients seeking remote care. This position provides direct care to patients seeking family planning, gender affirming care, and primary care. The APC works to assess the clinical needs of patients, in compliance with our medical standards & guidelines, including ordering, interpreting, and recording results of clinical tests. Prescribes medications and makes recommendations for other forms of treatment, including birth control.
As a member of the follow-up team, the APC works in conjunction with providers across the affiliate to ensure continuity of high-quality patient care by overseeing, executing and managing the follow up plans of care for all services provided at PPGP, thus ensuring an excellent patient experience. An unencumbered multi-state APC license is required for this position to allow for provision of services in the states in which PPGP operates; if APC does not currently have MSL, one must be obtained within 30 days of hire.
