Join our collaborative, patient-centered group of clinicians in the Metro Charlotte area, where supportive leadership, a strong work-life balance, and the opportunity to provide exceptional care come together.

Recognized by the Centers for Medicare & Medicaid Services (CMS) as a national model for improved care and reduced costs, the Post Acute Network Skilled Nursing Facility (SNF) Program provides on-site medical management through a dedicated Physician/APP team. The program focuses on preventing rehospitalizations and ensuring seamless transitions from the hospital to the SNF and from the SNF to home.

Position Highlights
• Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.
• Call: Shared call rotation
• Practice Model: In-person and virtual visits with Medical Assistant support
• Patient Panel: Complex patients seen 2–3 times per week
• Team Collaboration: Work closely with the SNF interdisciplinary team to coordinate care
• Discharge Planning: Implement and manage effective discharge plans to optimize outcomes
• Compensation & Benefits: Competitive salary with signing bonus, CME allowance, and relocation assistance

Ideal Candidate
• Licensed Physician Assistant (PA), AGNP, Adult NP, or FNP
• Passionate about working in a Skilled Nursing Facility (SNF) environment
• Experience in post-acute or SNF care preferred but not required
• Strong communication skills and team-oriented mindset
