Role Overview
Under the supervision of the Property Manager/Staffing Manager, the Service Coordinator will:
Coordinate programs and services that help residents age in place.
Serve as a liaison to community agencies and identify low-cost or discounted services.
Develop educational and wellness events, and implement on-site or mobile health screenings.
Engage residents to build support networks and connect with volunteers.
Provide individual support, assist with basic needs, and ensure residents receive desired community services.
Maintain all required documentation, care plans, and HUD reports.
What You’ll Do
Resident Programs & Activities:
Organize group and individual activities to promote health, social engagement, and independence.
Facilitate educational programs on healthcare, life skills, and community resources.
Connect residents with services including transportation, personal care, financial assistance, and more.
Customer Service & Advocacy:
Serve as the first point of contact for residents’ questions, concerns, and service needs.
Ensure residents’ rights are respected and report any complaints or compliance concerns immediately.
Collaboration & Documentation:
Work with healthcare professionals, family members, and staff to ensure residents receive the best care.
Maintain accurate records of services, referrals, and outcomes.
Other Responsibilities:
Support resident engagement in on-site and off-site activities.
Participate in training and professional development.
Perform additional duties as needed to support residents and the property team.
Qualifications
Bachelor’s degree in social work or related field preferred.
2+ years of experience providing social services to seniors or families.
Strong communication, problem-solving, and organizational skills.
Proficiency with Microsoft Office and general computer applications.
Valid driver’s license and ability to meet motor vehicle policy requirements.
Physical & Work Requirements
Frequent sitting, walking, standing, stooping, pushing, pulling, and light lifting (10–50 lbs).
Occasional climbing and lifting over 75 lbs.
Work autonomously and as part of a team in an environment with some distractions.
