RN Navigator (Southdale Cardiology Clinic)

Regardless of role, we recognize that every member of our team is vital! he RN Navigator - Cardiology accurately and appropriately provides patient-centered care by means of health protection, promotion, optimization and prevention of illness or injury. This position performs both independent and delegated nursing functions including: assessment, nursing diagnosis, planning, implementation and evaluation.

Our Fairview Southdale Cardiology Clinic supports a wide variety of patients with their cardiac needs.  This includes preventative care, coronary artery disease, hypertension, heart failure, atrial fibrillation and more.  We have a large group of providers that work closely with the RN's, clinical support staff, scheduling staff and more to provide excellent patient care.  Southdale was rated #1 in the twin cities for cardiac care, and coronary intervention.  We are excited to add an individual to our clinic who enjoys being part of a team and has a passion for caring for patients!

What you will do as a RN Navigator - Cardiology:

• Establishes measurable and achievable short- and long-term goals for patients
• Implements nursing protocols based on the treatment plan, ensuring continuity of care for patient
• Provides education to patient and caregiver(s)
• Evaluation of the patient’s status and efficiency of the nursing care and modified care plan as needed
• Helps provide healthcare access through nurse visits, management of in-basket task (phone messages, MyChart communication, refill requests, results)
• Provides safe and timely telephonic triage for symptomatic callers, using system approved triage policies and protocols

What you will need:

• Current Minnesota RN Licensure and knowledge of specialty area
• Associate’s (ADN) or Bachelor’s Degree in Nursing (BSN)
• 3-5 years of nursing experience within specific patient population group
• Patient education and care coordination skills
• Healthcare Provider Basic Life Support (BLS)
• Excellent assessment and care coordination skills

This position will be hybrid, and might require some travel to other site locations when in office.***

71,219.00 - 113,942.00 USD Annual
