Work Schedule: day hours, full time, 40 hrs./wk.

Provide onsite service coordination for the Wound Care Clinic. Role will include oversight of schedules, implementation of improvement projects, personnel management, payroll, fiscal accountability, and development of a skillful workforce.

Responsibilities:

Essential Job Functions

1. Provides on-site coordination of the Wound Care Clinic, by efficiently organizing a schedule to fit provider and patient needs, gather patient information and record follow up recommendations.

  • Preparation of schedules for supervising team and align schedules to meet needs of patients and clinic providers.

  • Coordinates/collaborates with ancillary departments.

  • Effectively organizes schedules and patient records for the clinics and conferences.

  • Prioritizes data collection according to patients’ immediate needs.

2. Assures appropriate staffing levels for patient care and operations.                  

  • Facilitates the hiring and training of staff.

  • Adjusts staffing appropriate to patient load.

  • Supervises and mentors clinic staff, including administrative personnel, nurses, and certified nursing assistants. .

  • Reviews time sheets on a weekly basis.

  • Monitors staff overtime.

3. Communicates with patients, families and referring providers to provide follow up information.

  • Provides timely communication with patients and families as needed.

  • Works collaboratively with other departments.

4. Ensures safety, competency and regulatory requirements are met.

  • Meets with Wound Center Medical Director monthly. Agenda and log required.

  • Collaborates in safety and regulatory activities such as TJC readiness activities.

  • Ensures wound care competency is completed annually by staff.

  • Ensures checklists are completed as required.

  • Reviews policies as requested.

  • Completes NOTEs (Notation on the Event) and follow up with appropriate action.

  • Participates in identification, analysis, reduction and evaluation of risk issues.

  • Monitors on an ongoing basis environmental safety issues and appropriate interventions.

  • Reports issues/trends to Service Executive.

5. Supervises and promotes a positive work environment that encourages employee development, employee involvement and employee satisfaction.

  • Establishes good rapport with staff members and peers.

  • Immediately addresses issues that may prevent a positive work environment.

  • Encourages staff to voice their concerns and works to address the issues.

  • Encourages staff to seek professional development and education.

  • Coordinates and leads meetings with direct reports no less than once per quarter.

6. Promotes a collaborative relationship with Medical Staff.

  • Establishes a good working relationship with the medical staff.

  • Seeks opinions of the providers on ways to improve operations.

7. Develops and implements the short- and long-range initiatives which are congruent with the hospital mission, vision, and core values.

  • Reviews previous and create department initiatives/goals on an annual basis.

  • Reviews the initiatives/goals with staff for input and acceptance.

  • Presents annual initiatives/goals to Service Executive.

  • Presents one performance improvement initiative.

8. Reviews financial spending in responsible areas to ensure fiscal responsibility.

  • Conducts monthly reviews of responsibility reports and addresses any variances.

  • Demonstrates responsibility in fiscal management of the departments.

9. Educates patients, families, and their caregivers as a contributor to the multi-disciplinary team supporting optimal patient care across the continuum.

  • Facilitates education to patients, and families about their treatment/procedure.

  • Supports each patient through by reinforcing education and coordinating information and care with a team of physicians and allied healthcare professionals.

10. Ensures the implementation of department policies and procedures encompassing the needs of the patent population are assessed, met, and evaluated.

  • Creates an environment to identify opportunities for and give individualized customer service/patient care according to customer/patient needs.

Job Requirements

Education:

  • Bachelor of Science in Nursing (BSN) required.

  • Satisfactory completion of annual mandatory education requirements required.

Experience:

  • Minimum of 2 years nursing experience required.

  • Previous clinical coordination or wound care experience preferred.

License/Certification:

  • Current valid Registered Nurse (RN) license, valid compact multistate license, or a temporary permit while awaiting licensure required.

  • Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.

Skills/Knowledge and Abilities:

  • Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel and co-workers.

  • Ability to multi-task and prioritize work assignments.

  • Ability to learn, retain, and utilize computer skills that are required for department, and hospital information system.

  • Computer knowledge with abilities in work processing required.

  • Knowledge of databases preferred.
