Job Description Summary:
MaineGeneral Health is seeking a compassionate and flexible Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team on a per diem basis. This is an excellent opportunity to work within a multidisciplinary team that provides person-centered, community-based care to adults living with serious and persistent mental illness.
The ACT Team operates 24/7, offering intensive, integrated support to help individuals live safely and independently in their communities. As an ACT RN, you’ll play a key role in medication management, crisis intervention, and ongoing care coordination — often meeting clients where they are: in their homes or in the community.
Job Description:
Position: Registered Nurse (RN) - ACT Team
Location: Augusta, ME
Schedule: Per diem/Flexible Hours
What You'll Do:
- Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors, records, and communicates patient condition as appropriate.
- Collaborates as needed across disciplines to coordinate patient care, including patient transfer, admissions, discharge, referral, and spiritual/psychosocial support needs.
- Evaluates the learning needs of the patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
- Effectively delegates and communicates patient care needs to ancillary personnel. May precept student nurses and new hires.
- Some employees in this job may operate a motor vehicle to conduct MaineGeneral Health business.
What You Bring:
- Current State of Maine RN license (or ability to obtain)
- Minimum of one year of nursing experience preferred; behavioral health experience a plus
- Valid State of Maine driver's license and reliable transportation required (travel within the community is part of this role)
- Strong communication and teamwork skills
