Registered Nurse - Patient Placement Coordinator Weekend Program Nights Friday/Saturday 1800-0630

Major Responsibilities:

  • Facilitates timely and efficient patient placement according to diagnostic acuity and site policy through communication with attending, consulting and referring physicians as well as with key departments, transferring and tertiary facilities. Provides the most medically appropriate placement based upon site admission triage protocol. In addition, keeps manager updated on critical issues related to high census, etc.
  • Works closely with patient placement clerical staff to facilitate communication with admitting physicians and patient care managers. Works with manager to resolve difficult issues as necessary.
  • Acts as a liaison resource for patients, physicians, patient care staff and families. Acts as a patient advocate by responding and intervening in patient and visitor concerns by using the appropriate resources to resolve the issues concerning bed placement.
  • Coordinates with Tertiary Access Program (TAP) in hospital-to-hospital admissions.
  • Works collaboratively with the clinical nurse specialist in the coordination of defined off-hour transfers to ensure appropriate and timely exchange of patient information and subsequent patient placement.
  • Reviews pending admissions and assists with processing patient admissions, transfers and discharges. Obtains patient's record from referral site and/or admitting/consulting physicians as appropriate.
  • Evaluates each transfer, hospitalization or discharge to identify any system issues.
  • Participates in Quality Improvement (QI) meetings related to Patient Access Services.


Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices.


Education Required:

  • Associate's Degree in Nursing.


Experience Required:

  • Typically requires 3 years of experience in coordinating patient care, utilization management, patient assess services and bed control.
  • Charge RN experience preferred


Knowledge, Skills & Abilities Required:

  • Intermediate computer skills.
  • Excellent communication (written and verbal) and interpersonal skills; ability to effectively communicate with a variety of patients, staff and physicians.
  • Demonstrated diplomacy and tact when working in stressful situations with all levels of staff.


Physical Requirements and Working Conditions:

  • Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders and twist frequently during work shift.
  • Must have functional sight, speech and hearing.
  • May be exposed to the following hazards on a continuous basis: mechanical, electrical, chemical, radiation, blood, and body fluids.
  • Will be required to wear gloves, goggles and gowns on an as needed basis.
  • Operates all equipment essential to performing the duties of the job.
