SummaryThe Pulmonary Program Coordinator role would be filled by a Respiratory Therapist, who would be working at top of license in terms of CPAP set ups, recommending adjustments to PAP settings, expertise in non-invasive ventilation. This position will offer the opportunity to improve care overall for CPAP/NIV patients.
Job Duties- Focuses in conducting remote physiologic monitoring treatment management services for sleep apnea (CPAP, Inspire) and other indications for non-invasive ventilation; work collaboratively with clinical staff/physicians/other qualified health care professionals, notifies provider of changes in patient condition or status, obtains new orders, and modifies the care plan as needed.
- Establish database of patients to be followed; collaborate with IT to create workflow/dashboard.
- Monitor PAP adherence and determine whether patients are in or out of range; respond to alert cases through remote monitoring tools.
- Engage in regular interactions/communication with patients/caregivers during the month, requiring both face to face evaluations, virtual visit evaluations, as well as phone calls.
- Provide patients monthly verbal assessments and health teaching (including appropriate use of PAP equipment) when clinically indicated, promotes patient self-management, provides education and disease processes, and empowers patients and families to achieve maximum levels of wellness, and health teaching.
- Identifies issues related to the healthcare system, transportation, financial, and/or psychological barriers and works collaboratively to problem solve, coordinate services, and to advocate for patients/families.
- Provides care management for the highest risk patients to reduce risk and unnecessary utilization to improve outcomes.
Minimum Qualifications- Associate’s Degree Graduate of a Registered Respiratory Therapy program accredited by the commission of accreditation for Respiratory Care
- 3 years experience in one of the following settings: disease management, home health care, hospital, DME or physician office setting. and
- 3 years Sleep Medicine or closely-related field of health care
- Demonstrates knowledge of and complies with applicable laws, regulations, guidelines, and standards as set forth by Regulatory agencies and the American Academy of Sleep Medicine.
- Strong verbal and communication skills in English.
- In-depth understanding of disease processes related to CPAP/NIV utilization, including but not limited to PAP adherence, PAP pressure adjustments, mask fittings and hospital to home sleep apnea care management.
- Time management and prioritization skills.
- Assessment and critical thinking skills.
- Clinical experience in with DME, sleep lab testing and inpatient settings is preferred.
- Knowledge of billing and responsibility for capturing appropriate billing charges under the remote monitoring program.
- Ability to incorporate strategies for interacting with persons from diverse backgrounds.
- Effective behavioral and educational strategies such as motivational interviewing, teach-back method, and self- management support.
- Ability to set priorities to coordinate care plan efficiently.
- RRT - Registered Respiratory Therapist - National Board for Respiratory Care Upon Hire or
- CRT - Certified Respiratory Therapist - National Board for Respiratory Care Upon Hire

Physical DemandsLift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
