Summary of Job Duties:

Join the UAMS Population Health Quality Team as a Population Health Improvement Assistant. In this role, you will support the Regional Population Health Improvement Manager (PHIM) and the Population Health Quality Team by performing administrative and documentation tasks essential to achieving Population Health, Value-Based Care, and Quality objectives.

Your responsibilities will include patient outreach and engagement, pre-visit planning, data tracking in payer portals, and other administrative processes to meet value-based requirements. You will also contribute to clinic operations through panel management, data collection, documentation for quality initiatives, and care gap closure. Additionally, you will assist in implementing standard work and best practices to enhance care delivery.

Qualifications:

Required Qualifications:

  • High School Diploma/GED
  • Two (2) years of experience years in a clinical or office setting doing work like scheduling, medical assistant, or administrative work.

Knowledge, Skills, and Abilities (KSAs):

  • Strong verbal and written communication skills.
  • Knowledge in basic medical terminology. 
  • Excellent interpersonal and collaboration abilities to work effectively with diverse teams and patients.
  • Advanced organizational and administrative skills to manage multiple tasks and priorities.
  • Proficiency in data tracking, documentation systems, and payer portals to support value-based care objectives.
  • Knowledge of population health management and value-based care principles to contribute to quality improvement initiatives.
  • Ability to collaborate within a multidisciplinary team to implement best practices and standard work.
  • Attention to detail and a commitment to maintaining high-quality standards in all tasks.
  • Proficiency in Microsoft Office Suite, including Word, PowerPoint, and Excel, for documentation and reporting.

Key Responsibilities:

Meeting Support and Documentation:

  • Assist in planning and organizing meetings, including preparing agendas, gathering input, and distributing materials.
  • Record, maintain, and distribute meeting minutes while following up on action items between meetings.
  • Coordinate meetings related to patient and family care, as well as the community care continuum.

Quality Improvement and Data Management:

  • Support the Population Health Improvement Manager (PHIM) and Quality Team by running reports and creating standard visual displays for designated locations.
  • Document outcomes and activities related to Population Health and Value-Based Care programs, including data entry in EMR or payer platforms.
  • Collaborate with the care team to identify high-risk patients with chronic care needs and ensure proper documentation for value-based care requirements.
  • Collect and organize data to support quality improvement, clinic operations, and internal audits.
  • Review payer reports, compare data with Epic, and complete required documentation or audits using standard workflows.

Patient Pre-Visit Support:

  • Provide administrative support to ensure pre-visit planning is thorough and effective, enhancing patient outcomes.
  • Assist with quality, patient experience, and safety-related administrative tasks.

Outreach and Patient Activation:

  • Address accessibility issues such as appointment availability, well visits, ED utilization, and MyChart access using standard workflows.
  • Conduct patient outreach to encourage engagement in care and connect patients with community resources as needed.
  • Communicate patient needs to the care team and reconcile external patient lists to support outreach, care gap closure, and outcome improvement.
