About the Role

An exciting opportunity currently exists for a Grade 2 Physiotherapist to join the Community Rehabilitation team on a permanent part time basis. The Physiotherapist will deliver high quality person-centred, goal-orientated home and centre-based care within a supportive multidisciplinary team.

This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

Your Contribution

  • Bachelor of Physiotherapy or equivalent

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation

  • Demonstrated knowledge and application of physiotherapy practice and principles in the area of rehabilitation

  • Demonstrated highly developed clinical skills 

  • Understand and support the principles of Health Independence Programs continuum of care for clients, particularly in relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes

About the Department

This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centered care planning.

St Vincent’s Hospital Melbourne (SVHM) HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH). This position is part of the HIP Community Rehabilitation team. Community rehabilitation is currently provided from our centers at Kew and Fairfield.   

The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness, are frail or chronically ill to regain optimal function and to allow people to maximize their independence. The programs offer a supportive and dynamic team environment, which focusses on patient-centered care.

1.LOCAL WORK ENVIRONMENT

Community Rehabilitation Services provide time-limited, goal–orientated care for people who have a physical disability, are frail, chronically ill or recovering from traumatic injury or illness to regain and /or maintain optimal function. Community Rehabilitation services comprise of Community Rehabilitation Centres at Fairfield and Kew and Rehabilitation in the Home.

This position may rotate through any of the Community Rehabilitation Services.

This position may also require working from any of the SVHM Health Independence Program (HIP) sites and may be required to work within any of the HIP programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH)

2.POSITION PURPOSE

To work within a flexible, interdisciplinary structure, that ensures the delivery of high quality therapy services to all clients of HIP Rehabilitation services, making sure that the client’s needs are met and they receive ‘the right treatment in the right place at the right time. Clinicians are required to provide assessment and management of clients in a number of settings across the continuum of care within HIP, including client’s residence, community settings, the HIP Centres or Specialist Clinics.

3.POSITION DUTIES

  • Assess and manage all assigned clients as appropriate, according to HIP and St. Vincent’s policies and procedures and relevant rehabilitation protocols
  • Deliver accountable, client focussed, high quality physiotherapy assessment, intervention, education and management
  • Communicate client issues effectively with other team members, with active contribution to multidisciplinary team meetings
  • Complete client goal setting in consultation with patient/family/carers as appropriate. Goals are to be functional and measurable
  • Evaluate treatment / management using the most appropriate methods, making use of objective measures where possible
  • Maintain current knowledge and skills in clinical best practice and rehabilitation
  • Participate in relevant in-service programs
  • Supervision of undergraduate allied health students as appropriate 
  • Provide guidance, support and direction to Allied Health Assistants within the HIP program as appropriate and on direction of senior Physiotherapy staff and/or Team Leader
  • Actively participate in quality improvement initiatives (includes working groups, projects, audits, discipline related projects) as directed by Team Leader/ senior physiotherapy staff
  • Maintain the appropriate standard of medical record keeping and data collection as per St. Vincent’s and HIP policies
  • Achieve set activity targets and compete activity statistics by end of working week or end of month (whichever comes first)
  • Participate in meetings as required and complete allocated tasks on time

4.INCUMBENT OBLIGATIONS

  • Complete required Fire and Emergency Training annually.
  • Complete required Workplace Culture and Equity Training annually.
  • Complete other mandatory training and locally determined requirements.
  • Attend general hospital orientation within 3 months of commencement
  • As required, comply with fit-testing and PPE requirements
  • Participate in reporting and analysis of safety and quality data including risks or hazards
  • Report any hazards, near misses and incidents (regardless of whether an injury has occurred or not) into Riskman
  • Identify and report any variance to expected standard and minimising the risk of adverse outcomes

6.SELECTION CRITERIA

6.1ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS

  • Bachelor of Physiotherapy or equivalent
  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Eligibility for Australian Physiotherapy Association (APA) membership

6.2 OTHER ESSENTIAL REQUIREMENTS

  • Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or community based service
  • A demonstrated sound knowledge and application of therapy practice and principles particularly in the areas of Orthopedic and Neurological Rehabilitation.
  • Demonstrated highly developed clinical management skills
  • Understand and support the principles of HIP continuum of care for clients, particularly in relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes
  • Highly developed leadership and interpersonal skills that incorporate excellent communication
  • Evidence of ongoing education/professional development relevant to Physiotherapy practice
  • Knowledge and commitment to quality improvement
  • Competent using Microsoft Office programs, database systems
  • Commitment to the values and health care philosophy of  St. Vincent’s
  • Current Victorian driver’s licence
