$24.85 - $37.30Major Responsibilities: Provision of physical therapy patient care activities 1)Performs various types of physical therapy techniques and procedures including ultrasound, hydrotherapy, exercises and gait. 2) Instructs patients and family members in activities of daily living and home exercise programs. 3) Recommends and with approval, orders equipment for patient care needs. 4) Responsible for meeting assigned patient care schedule. Documentation and maintenance of patients’ treatment records and charges. 1)Documents all treatment provided, patient progress, changes and response to treatment and all education provided to patient/family/caregiver. 2)Completes patient charges according REHAB services policies and procedures. 3)Completes and maintains patient care records according to REHAB Services policies and procedures. 4)Communication with physical therapists, physicians, nursing, and other personnel deemed necessary regarding patient care and status and appropriate family/caregiver. (3%) 5) Communicates with assigned physical therapist, patient progress and together resolves problems related to patient care activities. 6) Participates in patient care conference, interdisciplinary staffing as assigned. 7)Responsible for continuous monitoring and evaluation of own skills and techniques. (3%) 8) Attends all appropriate and recommended REHAB Services inservices and meetings. 9) Attends continuing education programs to enhance clinical expertise. Participation in departmental administrative processes, ongoing process improvement, quality assurance and safety programs 1)Provides data and/or input required for quality assurance indicators. 2)Participates on process improvement teams. 3)Evaluates environmental safety and provides environment that is safe based on age and development status. 4) Follows hospital infection control and safety protocols. 5)Ensures equipment calibration and maintenance procedures. 6)Completes patient billing process and follows scheduling procedure, as directed. Assumes other related responsibilities as assigned Licensure: Physical Therapist Assistant Education/Experience Required: Associates degree in Physical Therapy Assistant Recognizes needs and behaviors of specific age groups of patients treated. Knowledge, Skills & Abilities Required: Good Communication skills. General knowledge of treatment skills. Eligible for licensure to practice in Illinois Physical Requirements and Working Conditions: May be exposed to hazardous materials and life-threatening diseases Direct contact with body fluids Pushing, pulling, lifting, transferring patients greater than 25 lbs, as related to essential job activities. Ability to assist, escort, transfer, and respond to patient needs for extended periods of time. Services provided on weekends and holidays and during early evening hour Off campus locations This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
