order, track, and stock clinic supplies

*order, track, and deliver DME to participants as approved by the IDT

*pick up and deliver medications from contracted pharmacy to participant homes as needed

*deliver supplies to participants home as needed

Position Summary

The PACE Participant Coordinator is the liaison between the Outreach Coordinator, Washington State Home and Community Based Services, the PACE Center, prospective and active PACE program enrollees. The position is responsible for communicating directly with potential participants and their families to coordinate all aspects of the PACE program. The PACE Participant Coordinator is the electronic health record liaison for CMS and WA State compliance surveys and conducts routine compliance audits in the medical record. The incumbent collects, collates, and reports data regarding the major compliance areas of Service Determination Requests, Appeals, Grievances, and Quality Metrics. The PACE Participant Coordinator will recognize, and report established participant’s change of condition to the Clinical Director or designee. This position works cooperatively and collaboratively with management personnel to ensure smooth and efficient daily operations. Independent thinking, judgment, and decision-making skills are required within the scope of the position responsibilities.

Requirements

  • Bachelor’s or Associates degree in human services or administrative services preferred
  • COTA, PTA, CNA, or MA certification
  • BLS certification required
  • Minimum two (2) years of experience working in health care services in work related to coordination, communication, and direct patient care and services
  • Minimum one (1) year experience in medical clinic preferred

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $24.58 - $35.36 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
