Job Description:

Provide direct support to the SNF Care Coordination Department. Identify appropriate members for the Case Management program upon data screening and coordinate with team members to assure potential referrals are made. Act as point person to assist with SNF process related questions from internal support staff and customer service.  Resolve administrative problems by analyzing information; identifying and communicating solutions.

Job Responsibilities

  • Prepare reports by collecting, analyzing, and summarizing operational data and trends

  • Print daily report and open new members in AE utilizing the PMR report and attach MMH summary to all new cases

  •  Create reports for management to monitor engagement, and daily review and documentation of nurses queues to monitor production activity

  • Document all prescreening information in AE

  • Follow up with Provider’s offices to obtain member’s updated contact information

  • Interact with SNF staff members to enhance, coordinate and facilitate the overall triage process including triage appropriate pharmacy cases.

  • Maintain continuity of work operations by documenting and communicating needed actions to management; discovering irregularities; determining continuing needs.

  • Assist in the creation, updating and maintenance of SNF training tools on share point.

  • Update  visual boards

  • Tally survey Monkey results and create routine summaries of results

  • Assist with audit reports

  • Run reports for case selection

  • Answer SNF 800 line; check SNF Care Coordination mailbox daily and route messages.

  • Provide SNF members with basic information to assist in accessing appropriate services

  • Log into SNF Enhanced Care telephone and check messages and route as necessary.

  • Provide support which may include all of the following task types: agenda creation, document synthesized meeting minutes, create and track reports, creation of departmental tools and processes, ordering departmental supplies, booking travel and conferences, meeting scheduling and logistic setting. Record PTO days off for staff members.

  • Other support duties as assigned, including draft and review of internal and external communications, creation of meeting materials, and other responsibilities necessary to support department.

Required Experience

  • Minimum of 3 years of health care experience

  • Experience with Case Management

Required Education

  • Bachelor’s degree in Clinical Field, Health Administration, Management Information Systems, Business Administration, Communications, or related field, or equivalent experience.

Additional Information

Salary: $70,000 - $80,0000 Annual
