The Opportunity:
The Health and Wellness Manager is responsible for leading the clinical and wellness programs within the Retirement Home Community, ensuring high-quality resident care and creating a positive environment that supports resident well-being, safety and quality of life.
What Is In It For You:
Assess any ill residents in all areas.
Follow up on: Post Falls, Residents on Antibiotics, Behavioral Expressions, Documentation in Resident Progress Notes.
Communicate with Family members and/or Listed Contacts.
Oversee the assessment, planning, implementation and evaluation of residents’ care programs; ensure individualized care plans are current and adhered to.
Promote resident-centred care and a culture of wellness, dignity and respect.
Collaborate with physicians, allied health professionals, families and interdisciplinary teams to support resident outcomes.
Attend weekly management meeting: review new residents, hospitalized residents, infection prevention and control updates, quarterly IPAC meeting, care plan review, Care and Wellness Program status, Residents on Exit Plan, Current Issues-Residents, Families, Staff.
Weekly Supply Inventory (infection control, medical, housekeeping and laundry if applicable.
Weekly Care and Wellness Accountability-Submit to Executive Director.
Ensure staffing levels, scheduling, recruitment, orientation and ongoing education of care/wellness.
Ensure compliance with relevant legislation, standards, internal policies, accreditation and best-practice frameworks.
Collect and analyze data (e.g., quality metrics, incident reports, staffing statistics) and implement improvements.
Lead, coach, and mentor staff, fostering an inclusive team environment.
Conduct performance reviews and identify professional development needs.
Promote health & wellness among staff and residents.
Ensure a safe environment in compliance with workplace health & safety standards.
Implement wellness and activation programs supporting residents’ physical, emotional, social, and cognitive well-being.
Engage in risk management, incident investigation, and continuous improvement.
Ensures work performed is conducted in a safe manner in accordance with AgeCare policies and procedures including established Occupational Health and Safety procedures.
Works safely to reduce the risk of injury to self, other staff members and residents. Promptly reports all actual or potentially hazardous situations.
What You Will Need:
Registered Practical Nurse (RPN) with current registration in Ontario.
A current Certificate of Competence from the College of Nurses of Ontario and a member in good standing.
Minimum of 5 years’ relevant clinical experience in Long Term Care or Retirement Home or geriatrics; at least 1 year in a leadership role.
Demonstrated experience in program development, team supervision, budget management.
Knowledge of Retirement Homes Act, the appropriate sections of the Health Disciplines Act, Occupation Health and Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada), Coroners Act, College of Nurses of Ontario Standards of Practice and other pertinent Federal, Provincial and Municipal Statues which influence the operation of the Nursing Home and more specifically the nursing department.
A sound knowledge and background in geriatric, rehabilitation or long-term care/retirement home nursing including activation, nursing, and personnel administration.
