Health and Wellbeing Nurse – Forward Leeds

Location: This role will be based across sites, base location being Irford House, Seacroft, Leeds LS14 6PA and will involve occasional late night clinics and home visits
Working Hours: 37 hours per week, Monday-Friday with the occasional late night clinics until 7pm
Contract Type: Permanent
Salary: £40,266 - £54,478 per annum

About the Role

Forward Leeds have an exciting opportunity for a Health and Wellbeing Nurse, the role will support the clinical team in the service and delivery of direct service user care.  

What You’ll Do

  • To provide alcohol assessments, community detoxifications and referrals to in- patient or other support networks including mutual aid.  
  • To develop onsite service community alcohol detoxifications, working alongside operational colleagues.   
  • To work with hospitals in providing a managed pathway for people with alcohol misuse, through appropriate referral process or direct referral. 
  • To provide healthcare assessment across services but specifically in needle exchanges and outreach services.  
  • Provide a BBV service that incorporates screening and vaccination for Hepatitis B, Hepatitis C screening, re-screening and treatment provision in partnership with specialist services.
  • Co-ordinate and deliver call and recall services for Hepatitis B & Hepatitis A Immunisations, Hepatitis B, Hepatitis C and HIV Testing.  
  • Along with the wider teams work to promote harm reduction across needle exchanges, outreach services.

To Succeed in This Role, You’ll Need:

  • You will have an interest in bridging the gap between health and equality.
  • You will have a strong interest in working with vulnerable people.
  • Registered Nurse: You are a dedicated and registered nurse, either as a general nurse or psychiatric nurse, with the qualifications and commitment to provide exceptional care.
  • Clinical Expertise: You have experience in assessing clients to meet healthcare, clinical, and psychosocial needs, including addressing issues related to Blood-Borne Viruses (BBV). Your clinical knowledge is a foundation for providing high-quality care.
  • Health Promotion and Harm Reduction: You bring experience in health promotion and harm reduction interventions. Your ability to provide advice and information plays a crucial role in supporting individuals on their journey to recovery.
  • Driving licence and access to a vehicle is preferable due to working across various sites.
