Position Title: Case Manager (First Response)
Classification: SCHADS Crisis Level 2 / Social Level 4
Location: Social Mission Homelessness – Spring Hill
Status: Permanent Full Time | Monday-Friday | 8:30am to 4:30pm

We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.

About the role

The Case Manager (First Response) plays a key role in delivering high-quality, person-centred support to individuals and families who are experiencing or at risk of homelessness across South-East Queensland.

This position is responsible for providing both over-the-phone and face-to-face interventions, including triage, assessment, interim response, and case management. Clients receive initial assessment and referral, followed by the development and implementation of person-led case plans that promote sustainable outcomes.

How You Will Make an Impact

  • Deliver services in alignment with the National Homelessness Stream Model of Care, ensuring quality and consistency in all interactions with clients, partners, and stakeholders.
  • Respond promptly to phone and email enquiries, providing interim support, assessment, and triage of needs, risks, and vulnerabilities. This may include completing QHIP assessments, AVHHTS, safety plans, and other relevant tools.
  • Develop person-led case plans in collaboration with clients, identifying goals, needs, and barriers, and implementing targeted interventions to achieve positive outcomes.
  • Maintain accurate and up-to-date client records and data in accordance with TSA standards, using both TSA and government information systems.
  • Flexibly work across multiple program areas as required, in response to emerging community needs, trends, and organisational priorities, as directed by the Team Leader.

What You Will Bring

  • A degree qualification in Social Work, Welfare, Community Development, or a related field; or extensive sector experience with demonstrated skills and knowledge gained through previous roles and professional development.
  • Relevant experience in a social services setting, including client assessment and case management.
  • Queensland Working with Children Check  (Paid Blue Card)
  • Current Driver’s Licence.
