Position Purpose
Coordinates services to support Community Health & Well-Being initiatives. Supports program initiatives by collecting local data, ensuring accuracy of the data, and communicating data to system leaders. Coordinates collaborative work with internal and external stakeholders.
What you will do
Support the development of triennial Community Health Needs Assessment (CHNA) including convening stakeholders, developing survey materials, collecting and analyzing data, and writing reports
Collect, enter, review and assess the Community Benefit expenditures and outcomes data provided
Assist in planning and implementing community benefit and CHWB programs in response to the CHNA and Trinity Health strategies
Support strategies regarding Anchor Institutions (i.e., local hiring and local sourcing) and policy, systems, and environmental change initiatives in collaboration with internal and external partners
Manage timely and accurate coordination for external regulatory requirements; populate and review Ministry Schedule H submission
Work with the finance liaison to accurately calculate community benefit expenditures
Regularly create reports through CBISA and apprise Ministry CHWB management of community benefit spending and outcomes
Minimum Qualifications
Bachelor's degree in Health Care Administration, Public Health, Community Health or a related field with one (1) to three (3) years of experience community health, public health, social services, or an equivalent combination of education and experience
Strong data entry skills and project management skills
Excellent communication (interpersonal, verbal and written) skills
