About the Role:

  • Deliver high-quality mental health interventions within a multidisciplinary community setting, supporting clients with complex needs including mental illness, substance use and homelessness using a recovery-focused framework.

  • Lead, coordinate and direct client care, ensuring treatment is responsive, holistic, and aligned with individual goals and support networks.

  • Maintain accurate and timely case records, documentation, and data reporting to meet clinical, legislative, and service requirements.

  • Actively monitor, manage and escalate changes in client risk status to ensure safety, appropriate intervention, and continuity of care.

  • Collaborate effectively with internal teams, external service providers, families, carers, and community networks to support integrated, client-centred outcomes.
     

About You:

  • Relevant qualifications in Nursing, Psychology, Occupational Therapy, or Social Work, with full AHPRA registration (or AASW eligibility for Social Workers), and a current unrestricted NSW driver’s licence.

  • Demonstrated experience working within a Community Mental Health position as part of a multidisciplinary health care team.

  • Demonstrated ability to deliver high-quality client care and services that support families and carers of people living with mental illness.

  • Committed to the provision of recovery-focused, holistic mental health interventions.

  • Highly organised with excellent computer, written and verbal communication skills, effective interpersonal and organisational capability, including confidence using an electronic medical record system.

In this role, you will be based at St Vincent’s Hospital, Darlinghurst.
