Community Health Worker
Based in Portland, OR
**This is a field-based position with frequent in-state travel required, with approximately 70% of each day spent in the community visiting members. Must have reliable transportation and be located in Portland, OR to considered for this role.**

As a member of the Care Advocate team, the Community Health Worker participates in the integrated approach to care management for members. This role provides outreach as part of specific care gap initiatives or as an on-going part of a care management team. The position works with members to achieve their personal health outcomes and support successful transitions of care by removing barriers, facilitating medication reconciliation post-discharge, and providing support and education.
The Community Health Worker promotes successful inpatient hospitalization discharge to the community by collaboration with hospital discharge management staff, Cambia utilization and case management staff, and community-based organizations. This position encourages member self-activation with their health care goals and provide appropriate referrals to internal or external services and resources – all in service of making our members’ health journeys easier.

Qualifications:
The Community Health Worker would have an Associate's degree or clinical certification such as a CNA, Medical Assistant, and 5 years of health-related experience including at least 2 years of Care Advocate or similar experience or an equivalent combination of education and experience. While not required, a Community Health Worker Certification is preferred.
Candidates on the OIG Medicare exclusion list will not be considered for this role.

Strong customer service skills including listening, patience, empathy and maintaining confidentiality.
General organization, data entry and administrative skills.
Proven ability to effectively handle multiple tasks and prioritize workload.
General computer skills (including use of Microsoft Office, Outlook, Internet search).
Ability to work a varied schedule such as early evening.
Basic knowledge of medical terminology and familiarity with topics such as specific disease and lifestyle topics and related resources.
Ability to work independently, to prioritize work, meet deadlines and achieve operational standards.
Ability to research and problem solve in a creative manner to reach a solution.
Ability to identify when it is appropriate to refer to others.
Knowledge of health-related topics and a general understanding of clinical services programs such as Utilization Management, Care Management and Disease Management. Typically demonstrated by at least 2 years of successful Care Advocate experience and ability to handle a wide range of member issues or interventions.
Proven ability to effectively engage and motivate others to take action regarding their health.
Proven ability to work collaboratively in a team environment and actively contribute to a team success.
Ability to understand clinical documentation such as hospital discharge plans or clinical progress notes to make appropriate care management referrals.
Demonstrated ability to be pro-active such as taking on additional projects and continuing to develop skills.
Experience with AI tools and technologies to enhance productivity and decision-making in professional settings highly desired
What You Will Do at Cambia:
Initiates outbound telephone calls or emails to members identified for the Regence Care Management Programs for defined care gap initiatives.
Utilizes motivational interviewing and behavioral change principles to encourage engagement in health-related activities and programs.
Educates members by providing the tools and information they need to adhere to nationally recognized preventive screenings, discharge and treatment plans.
Makes referrals to licensed clinical staff as appropriate.
Utilizes the applicable care management platform to appropriately record and track member interactions.
Initiates home or community/hospital visits with members both pre and post-discharge and provides members with support, encouragement, and education to improve the member’s ability to make informed healthcare decisions, navigate care options and to take actions in order to improve well-being and prevent/minimize disease, and follow through on discharge orders.
Works with the member, members’ families, caregiver(s) and providers in order to facilitate a successful discharge to the community.
May assist with activities in support of the Clinical Services Team, including but not limited to program communications and training.
Identifies areas in need of improvement in all aspects of program and provide recommendations for solutions.
Supplies Subject Matter Expertise on program processes, procedures, and deliverables.
Work Environment
Approximately 70% of each day will be spent in the community visiting members.
When not working in the field, this position will be fully remote.
Work primarily performed in the home-office environment, conducive to performing confidential telephonic meetings.
May be required to work alternate work schedule or overtime.
May be required to work outside normal schedule.
The expected hiring range for a Community Health Worker is $28.00 - $35.10/hour depending on skills, experience, education, and training; relevant licensure / certifications; and performance history. The bonus target for this position is 5%. The current full salary range for this role is $28.00 - $45.70/hour.
