Community Health Worker (On-Call, Float Position)

Location: Multnomah County Health Department – Portland Metro Area

Sites Covered: 7 Primary Care Clinics

Overview:

Join a team that makes a real impact. As a Community Health Worker, you will support individuals and families by addressing social determinants of health, connecting them to essential services, and promoting overall well-being. This role blends community outreach and clinical collaboration to bridge gaps in care and improve access to resources such as food, housing, financial assistance, and health education.

Key Responsibilities:

  • Provide referral outreach, care coordination, and direct client support in both clinic and community settings.

  • Conduct home visits to assist clients with accessing services, addressing health concerns, and navigating complex systems.

  • Deliver interactive health education and promotion activities such as walking groups, cooking classes, and wellness workshops.

  • Collaborate with care teams, document client interactions in Epic, and maintain communication with providers.

  • Build and sustain partnerships with community-based organizations and diverse populations.

  • Foster a culture of equity, respect, and inclusion within the community and workplace.

 Minimum Qualifications/Transferable Skills: 

Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.

Education/Training and Experience: 

Equivalent to an associate's degree and one (1) to four (4) years of experience in community outreach services providing health information, advocacy, social support and assistance in using the health care system to groups and families; and experience working with the community being served.

Other: 

  • Must have a valid Driver's License 

  • Must have valid CPR / BLS certification upon hire OR within 30 days of hire. 

Preferred Qualifications/ Transferable Skills: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.   • Community Health Worker (CHW) Certification is preferred and may be required for this position in the future. If applicants do not already have the CHW certification, there is the ability to gain the certification during employment.

Be sure to describe any transferable skills on your application and clearly explain how they apply to this position.
