JOB SUMMARY
Under the direct supervision of a Portfolio Clinical Lead, the Clinical Resource position will support local clinical leaders at agencies located throughout Connecticut. In association with those local clinical leaders, the Clinical Resource will be responsible for supporting the local agencies and field staff. May also be utilized for education and operational assistance in temporary DCS role and back-office support.

DUTIES & RESPONSIBILITIES
Responsible for providing a high level of support and guidance in the establishment of programs, policies and practices that ensure that all agencies provide exceptional clinical care in compliance with State and Federal Regulatory requirements.
Educates and assists local clinical leaders to ensure field clinicians are working at the highest level. Works closely with compliance resources to ensure appropriate and effective survey preparedness.
Assists local leadership in developing and implementing clinical processes to monitor and measure all activities related to clinical, education, and performance improvement.
Provides support regarding the direction and coordination of educational programs, orientation, new hire competency systems, annual skills competency and on-going in-service educational program development.
Helps develop initiatives, monitor agency processes and progress in areas including patient safety, risk management, and infection control programs.
Educates and trains clinical supervisors and managers to ensure documentation is complete and consistent with legal requirements. Additionally, a clinical resource may be asked to step into patient homes for observations, evaluations, and mock surveys.
This position is office and field based and may need to be in a branch across the state of Connecticut for support up to 5 days a week. Occasional travel to meetings or other Portfolios across Cornerstone/Pennant may be needed.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Qualifications
• RN, PT or OT
Minimum Experience
Current unrestricted Connecticut license as RN, PT or OT.
A minimum of five years’ recent Home Health and/or Hospice experience
Home Care Home Base EMR experience preferred
Understanding of the legal and regulatory framework governing the home health and hospice industry
Ability to work with all levels of management.
Will be required to show proof of a valid driver license, motor vehicle insurance, and a 2-step TB screen (giving within the past 6 months of date of hire).
Ability to work with all levels of management on all aspects of clinical operations
Verbal and written communication skills necessary to explain complex and/or confidential information.
Time management and organizational skills necessary to manage multiple projects, appropriately prioritize workload, plan for agency leaders to meet deadlines and goals, and work independently in responding to day-to-day functional needs.
Demonstrated ability to interact effectively with employees.
Ability to anticipate and identify problems and use sound judgment and fact-based analysis to develop effective and efficient solutions.
Software skills, including the use of Microsoft Office software and web-based applications.

Licensed Registered Nurse or licensed therapy discipline (PT/OT/ST)
Minimum 5 years of home health and/or hospice experience
Experience in agency-level clinical leadership
Solid understanding of home health and hospice regulations
Strong communication, coaching, and time-management skills
Confidence to work independently and collaboratively across organizational levels
HCHB experience preferred
