Job Summary: The clinical pharmacist assigned to the outpatient pharmacy performs with reviewing, interpreting, and verifying medication orders for appropriateness in a timely manner; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care.

Major Duties:

Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria. Approves or denies non-formulary requests per policy and evidence-based criteria. Provides information on therapeutic substitutions when non-formulary consults are denied. When approved, performs therapeutic substitution as indicated. Actively involved in taking health and medication histories and performs medication reconciliation by reviewing patient medication regimens for clinical effectiveness drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Applies knowledge of normal laboratory values and physical assessment information in the evaluation of pharmaceutical care. Recognizes significant deviations from normal parameters and can identify and solve medication-related problems based on objective findings Serves as an authority on medication utilization, drug-drug and drug-food interactions, acceptable medication dosages, and compliance strategies. Educates medical staff, nursing staff, pharmacy technicians, and students on medication management. Uses appropriate references to research drug information. Functions as an available preceptor for SAVAHCS pharmacy residents, performing medication use evaluations (MUEs) when needed. Assures safe and effective medication utilization by identifying and resolving medication related concerns. This review includes a broad understanding of pharmacy practice, including but not limited to pharmacotherapeutics, pharmacoeconomics, and pharmacokinetic knowledge derived from evidence-based resources. Communicates findings and recommendations with the medical care team. Participates in training all new pharmacy staff members to the functions for the outpatient pharmacy areas. Directs and supervises pharmacy residents and students as assigned. Participates in activities of interdisciplinary training and in-servicing of medical staff, nursing staff, residents, students, and other allied health professionals at SAVAHCS as required. Effectively instructs patients and family members in the appropriate use of medications. Adjusts communication and tracking methods based on age and developmental considerations. Accurately assesses and documents patient comprehension. Refers patients to educational programs provided at the medical center by multidisciplinary teams. During all patient interactions, ensures that the patient medication list is up-to-date and explains the importance of maintaining an up-to-date list. Complies with HIPAA, VHA, Medical Center and Service/Department information, computer, and facility-related security regulations and policies/procedures. Safeguards own computer access and verify codes and logs off computer when leaving work area. Preserves the integrity of the Pharmacy Service security by complying with security policies and safeguarding security codes. Work Schedule: Rotating Shifts Monday through Friday between 7:00AM and 6:00PM, with rotating weekend (Saturday/Sunday 8:00AM - 4:30PM) and holiday coverage. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the 2 graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements, the following criteria must be used when determining the appropriate grade assignment of candidates. GS-12 Clinical Pharmacist Education and Experience. (a) 1 year of experience equivalent to the next lower grade (GS-11), or (b) completion of an ACPE-accredited PharmD. Program. Demonstrated Knowledge, Skills, and Abilities (KSAs). Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
