Clinical Pharmacist Advisor - Cigna - Remote (FL, GA, MS, TN, TX or VA)

This is a remote position. Ideal candidates must reside and be licensed in one of the following states: Florida, Georgia, Mississippi, Tennessee, Texas or Virginia.

This is a full-time work schedule, Monday- Friday with an 8-hour dayshift CST during business hours. Candidates must be flexible to work outside of these hours to accommodate member availability and outreach windows.

This position is responsible for providing targeted Medication Therapy Management (MTM) services to poly-chronic, high-cost members within the Individual and Family Plans (IFP), many of whom face significant social determinants of health challenges. The Clinical Pharmacist will focus on direct member outreach and clinical engagement, applying creative problem-solving and clinical judgment to improve medication outcomes and support care transitions, where applicable.

This is a hands-on clinical role. The ideal candidate is nimble, curious, and energized by member-facing work that requires adaptability and thoughtful clinical action.

Key Responsibilities

  • Conduct proactive outreach to IFP members identified via claims, discharge data, or care coordination triggers, focusing on polypharmacy, adherence gaps, and high-risk medication use.

  • Deliver Comprehensive Medication Reviews (CMRs) and develop personalized Medication Action Plans (MAPs) that address clinical and socioeconomic barriers.

  • Collaborate with multidisciplinary care teams to ensure continuity of care and timely resolution of medication-related issues.

  • Document clinical interventions and member interactions in designated platforms, maintaining accuracy and compliance.

  • Support outreach campaigns such as 90-day conversion, generic optimization, and readmission prevention through direct member contact and follow-up.

  • Apply creative thinking and clinical judgment to navigate complex member scenarios and adapt outreach strategies to meet evolving needs.

Required Qualifications

  • Doctor of Pharmacy (PharmD).

  • Active pharmacist license in applicable state(s).

  • Minimum 2 years of clinical experience in ambulatory care, MTM, or managed care settings.

  • Strong interpersonal and communication skills, with a demonstrated ability to engage members and collaborate across teams.

  • Experience with pharmacy analytics platforms preferred.

  • Familiarity with IFP population health strategies and QRS-aligned clinical quality measures.

  • Bilingual in Spanish a plus
