Clinical Documentation Specialist

Job Description Summary:

Clinical Documentation provides a formalized approach to promote accurate and complete medical record documentation reflecting a patient's true severity of illness and risk of mortality. The CDS reviews ambulatory medical records and identifies opportunities in provider documentation that will clarify patient severity of illness, risk of mortality, quality indicator measures and intensity of care rendered.

This role will be responsible for leading the development of education materials and education sessions with providers and staff. This includes both OhioHealth Physician Group (OPG) providers, as well as Independent providers in the Clinically Integrated Network (CIN).

Responsibilities And Duties:

50%
Develops education materials through research of best practices and up-to-date government requirements.
Collaborates with other Clinical Documentation Specialists (CDS) to create tip sheets and update on a yearly basis.
In-person/virtual education sessions with CIN providers and staff on topics of focus for the department.
Presentation of learnings and proposed improvement opportunities.

30%
Support the Clinical Documentation Specialist (CDS) team with ambulatory chart reviews.
Utilize department tools to locate opportunities for improvement on coding and documentation.

20%
Exhibit expert knowledge of clinical documentation requirements and coding guidelines.
Understand compliance standards and comply with AHIMA Standards of Ethical Coding guidelines.
Demonstrate competency of ICD10 documentation requirements.
Possess working knowledge of computer-based applications including but not limited to Word, Excel, and facility-based Electronic Health Record (EHR).

Minimum Qualifications:

Diploma: Nursing
