JOB DESCRIPTION:

SUMMARY OF POSITION

The community outreach specialist is responsible for developing and implementing outreach strategies that increase awareness of Centerstone services to community members. This role builds and maintains relationships with community members, attends events, and conducts comprehensive outreach services to include intervention, education, and linkages to services. Meets with individuals and community partners in community settings - hospitals, community agencies, etc.- and ensures continuity of care. Screens and assesses potential participants and assists them in determining eligibility for organizational programs or other forms of assistance. The community outreach specialist takes initiative in coordinating outreach activities and evaluating community engagement outcomes under direct supervision of the CCBHC Manager.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Knowledgeable on Centerstone programs and services, eligibility requirements, and availability of services with the ability to speak in depth about each program and how to begin services.
  • Ensures continuity of care for clients by coordinating with team members, families, Centerstone colleagues, and community partners
  • Increase contacts with community organizations to increase client referrals and maintain relationships with the goal of encouraging use of the organization’s programs.
  • Complete the enrollment and assessment process for eligible participants interested in Centerstone’s programs, maintaining accurate records and documentation.
  • Conduct ongoing assessment and evaluation of community needs and changes, adapting outreach strategies accordingly to better serve the community
  • Implement strategies to inform the public about the mission of the organization and creates relationships to successfully develop CCBHC clinics
  • Responsible for the development of outreach strategies that increase awareness of Centerstone services to community members by implementing community education strategies such as attending local events independently, distributing materials, leading workshops/educational sessions, and collaborating with partners.
  • Required to work with independent discretion and resourcefulness and seeks out additional duties as applicable.

KNOWLEDGE, SKILLS & ABILITIES

  • Knowledge of and skilled in community engagement strategies, relationship-building, and networking
  • Skill in customer service techniques.
  • Skill in responsible delegation, time management and prioritizing tasks.
  • Skill in Microsoft Word, Excel, PowerPoint, Outlook, and Electronic Health Records
  • Knowledge of clinical best practices.
  • Knowledge of confidentiality guidelines.

QUALIFICATIONS

Education Level

  • Minimum requirements for education: Master’s Degree in Human Services Field, licensure eligibility required

Years of Experience

  • One year of experience providing behavioral health services required
  • One year of experience working in community outreach or similar field preferred

$43,900.00--$61,370.00
