Position Summary:
Nurse Case Manager is responsible for assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long- term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

This role requires work at home as well as field based work environments. Typical office working environment with productivity and quality expectations. Work requires the ability to perform close inspection of hand written and computer generated documents as well as a PC monitor. Sedentary work involving periods of sitting, talking, listening. Work requires sitting for extended periods, talking on the telephone and typing on the computer.
Candidate will work from home (WAH) out of their residence in VA
Have reliable phone and hard-wired internet service.
Work schedules are determined by the Leader based on business needs and are not affected by the work model or location. Mon-Fri 8-5 PM EST
Colleagues must be reachable via phone, videoconference, and email during work hours (8 Hours per Day) as directed by their Leader.
Personal responsibilities, including dependent care, must not interfere with work duties. Dependent care during work hours is generally not allowed.
Be secure and private to maintain confidentiality (HIPAA compliant).
Meet any additional reasonable requirements set by the Leader or Company.
A field-based Colleague regularly works in multiple company locations. Those locations may include company locations, client locations, facilities (nursing home, rehabilitation centers, etc.), member homes, and/or community locations.
Preferred Qualifications:
Case management and discharge planning experience
Managed Care experience
Education:
Associates Degree In Nursing Required.
Prefer a BSN
Technological Skills:
Proficient in Multisystem Navigation: Comfortable using multiple monitors and systems simultaneously to streamline tasks and improve efficiency.
Effective Multitasker: Skilled at talking and typing concurrently while managing various responsibilities with focus and accuracy.
Basic Technical Troubleshooting: Able to set up and connect essential hardware (monitors, keyboards, etc.) and resolve common technical issues independently.
Digital Collaboration & Documentation: Experienced in electronic documentation and virtual collaboration tools to support remote teamwork and communication.
Highly Organized & Results-Oriented: Maintains a structured approach to work, ensuring tasks are completed on time and in alignment with performance metrics.
Metric-Driven Performance: Demonstrates accountability by consistently meeting or exceeding established goals and expectations.
Clean & Efficient Workspace Management: Keeps workspaces tidy and organized to support productivity and comply with professional standards.

The typical pay range for this role is:
$60,522.00 - $129,615.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
