Key Position Details:

Job Description:

Responsible for Care Management project management and consulting on programs and project improvement initiatives that support the Care Management strategic plan.

Principle Responsibilities

  • Leads in the advancement of Care Management operational initiatives
    • Guides workflow improvements or optimal performance delivery.
    • Partners with leaders to plan and carry out the Care Management strategic plan.
    • Partners with clinical experts in developing appropriate clinical best practice standards for targeted populations. Assists in the implementation.
    • Assists in identifying unnecessary variation in clinical practice that may impact quality, accreditation, and total cost of care.
    • Assists operational leaders in the implementation and sustainability of plans.
  • Project Management
    • Organize complex projects, teams and initiatives.
    • Collaborates with clinical and non-clinical leaders to formulate and evaluate project tactics.
    • Uses quality improvement tools to facilitate projects within defined deadlines effectively.
    • Drives results to improve targeted performance indicators, including Length of Stay and financial performance.
    • Identifies and presents evidence-based practice to key stakeholders, including clinical and non-clinical staff members and site leaders.
  • Applies analytical skills to analyze and present performance measures
    • Offers informed consultation for data-driven decision making.
    • Analyzes and presents performance data in alignment with Care Management's key strategic objectives.
    • Conducts investigations and facilitates reviews of lagging performance or regulatory deficiencies and advises on improvement strategies.
  • Serves as a connector with big-picture lens across the organization
    • Brings people and workflows together.
    • Serves as a mediator between technical and operational teams.
    • Works cross-functionally and cross-programmatically to unify tools and workflows.
  • Advises use of analytic tools that support evidence-based best practices
    • Advises operational leaders on leveraging EPIC functionality to its fullest.
    • Provide knowledge and insight into operationalizing regulatory accountabilities.
    • Compliance monitoring through auditing.
  • Other duties as assigned.


Required Qualifications

  • Bachelor's degree in Nursing, Public Health, Pharmacy, Health Care Administration, Engineering or other health care related field
  • 3 to 7 years experience in health care, project management, performance improvement, quality improvement and/or patient safety
  • 2 to 5 years experience in electronic medical records
  • 2 to 5 years experience in project management and/or quality improvement principles


Preferred Qualifications

  • Master's degree in Nursing, Public Health, Pharmacy, Health Care Administration, Engineering or other health care related field


Licenses/Certifications

  • Certified Professional in Healthcare Quality (CPHQ) preferred
  • Health Care Quality Management (HCQM) preferred
  • Certified Professional in Patient Safety (CPPS) preferred

Pay Range

Pay Range: $41.98 to $58.08 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
