Provides health care services to patients under the oversight of a physician. Follows established standards, procedures, protocols, and practices, and gives specific patient care directions to nursing and other staff . This position requires the ability to assess, diagnosis, treat, care for, understand and react effectively to the unique needs of the patient population for whom services are provided . Must be able to demonstrate the knowledge and skills necessary to provide care and service to patients age 18 and above.
PATIENT CARE:
- Responsible for the medical management of assigned patient population including assessment, diagnosis, intervention, appropriate referral for care, and outcome evaluation.
- Performs physical examinations, conducts approved diagnostic and therapeutic procedures.
- Secures health histories, orders and interprets diagnostic tests, records findings, makes critical assessment of patient needs and develops, reviews, and modifies treatment plans as required.
- Prescribes medications as allowed by pertinent Michigan regulatory agencies.
- Collaborates with consulting physicians and other members of the health care team.
- Maintains the continuum of care through the coordination and integration of all phases of patient care using physician orders, progress notes, and routine orders/best practice protocols.
- Independently accesses changes in patient status and institutes routine and emergency interventions to stabilize patients including those in the ICU & Telemetry.
- Performs procedures delegated by supervising physician including insertion of arterial and central venous catheters, suturing, intubation and ventilator management.
- Provides essential functions to inpatients managed by the adult Hospitalist service as needed.
- Determines patient/family educational needs and provides education to meet the identified learning needs. Documents the provided education and plan in the patient’s chart.
- Facilitates discharge planning along with the patient, family and other members of the healthcare team.
BEHAVIORAL HEALTH ASSESMENT AND TREATMENT COORDINATION:
- Maintains systematic, timely and legible documentation in the medical record following Holland Hospital and Behavioral Health Service policies.
- Completes documentation of a medical history and physical examination for all patient’s admitted to the inpatient behavioral health unit within 24 hours of admission.
- Communicates clearly and proactively with supervising physicians and therapists who share involvement in a case.
- Refers patients requiring medical management and other services appropriately.
- When appropriate, maintains communication with primary care physicians to facilitate coordination of care.
QUALITY IMPROVEMENT/RISK MANAGEMENT:
- Ensures provision of high quality of care.
- Participates in problem identification and continuous quality improvement related to clinical services and coordination of care as well as program/staff growth and development.
- Maintains current knowledge related to practice excellence in specialty area.
- Attends educational programs, completes mandatory educational requirements, and reads appropriate professional literature to maintain and enhance skills and clinical competence.
- Routinely consults with appropriate administrative staff about incidents that potentially impact risk management to determine an appropriate action plan .
ADMINISTRATIVE:
- Performs duties in accordance with medical staff bylaws, policies and the standards of accrediting agencies such as DNV, third party payers as well as federal, state and local government laws, rules and regulations.
- Maintains records, charts, report and forms in conformity with standards and policies.
- Assures documentation meets all requirements for a complete and accurate medical record.
